There are two ways to open an account at Wade Assurance.
1.  Completing an account online
     
Opening an account online is probably the easiest way to get the account opened.  This        starts with letting me know you would like an application.  RJ O'Brien will forward you a        link to begin the account application process.
2.  Downloading an application
     Download the forms necessary to open your account. Complete and send your                    application to me via fax, email or mail.

New Account Documents

Please fill out the Account Opening Booklet. In addition to the account form booklet you must also download the disclosure booklet and sign the acknowledgment at the end of the booklet along with your account forms.  I will also need a photo of your drivers license and a photo of a voided check.

Funding The Account
Wade Assurance funds brokerage accounts easily through ACH withdrawals.  Complete page 30 of the individual account application or page 32 of the other entities account application.  The ACH allows us to easily move funds in and out of your account with your permission. 

​Subscriptions are available on a monthly basis for $50 per month.  
Steve Wade and Tyler Wade of Wade Assurance are associated persons for AgDairy LLC.
